Probably left hand threads. He already has 2 of them off.
Common on older and heavy duty vehicles, one side only.

You are 100% correct. When I worked at a trailer shop 15+ years ago, the older trailers with split ring rims had those backwards left handed threads. They’re called Budd nuts or something like that. I think military vehicles use them too.

Dodge and Chrysler had LH threads on the passenger side until the mid '60s I believe...
