Ahh a follower jumped out of place. That could have been caused from the timing being jumped out of place,the chains, gears, and tensioners all look in place and in the right spots?

Either a valve laid down on you (sticking and not fully coming back to the seat) or the lash adjuster isn't staying pupmped up. Compression check once the follower is back would tell you if something's amiss.

Bob is more correct. If the lifter collapsed or is not staying pumped, for surely the follower would get thrown out of there...
