Well normally if it was an issue with the cats the rear 02s would throw some sort of code as well because their only purpose is to monitor the cats performance.

I'm going to ask a strange question. Have you had your transmission out lately, or been messing with the speedo? My car was giving similar codes and it turned out one of the speed sensor plugs on the tranny had a wire pin pulled out of the plug. It was sending voltage back thru the system and messing up the mixture. The dealer said the code was saying rich when in fact it was actually running lean. I know nothing about this, I'm only repeating what they found but, your situation sounds similar so I thought I would pass it on.

its never been to a tuner, the tranny was out, but that was like 8 months and 6k miles ago (ill check that out tonight), plugs dont look bad, ill look at the connection on the tranny, and i got got behind it this morning in my mach and had my woman take off hard and no black smoke, when i put my hands over the tail pipes there is hardly any pressure comming out, not as near as much as my mach1.

4a7191a, I am not sure on that but cleaning your MAF certainly would not hurt.
Perhaps some here that do know will get around to responding.

now she just called me, and at a red light with a smooth idle it idled down and died, but it started up fine, and runs good still, she said it did this twice.

By chance, have you cleaned the MAF yet?

I found some material online that indicated that a fouled or 'dirty' MAF could throw a "System too rich" code for either bank.
DRIVING CONCERNS
The function of the MAF is described in Module and that it needs regular cleaning is in Clean MAF. However, sometimes cleaning the MAF does not produce the right result. On the Scorpio automatic, a dirty or failed MAF can produce the loss of 4th gear engagement, incorrect ratio errors and TCC errors, often associated with brief flashes from the TIL light. But if after cleaning it, how do we decide if the MAF is faulty when the faults persist? One owner of a 16v (Simmo) reports that total failure of the MAF sensor made the engine race, then stall, so if you get this symptom the MAF is the first thing you should check.

Ford advice details some or all of the following concerns :

poor driving behaviour
engine shudder
engine ringing noises
engine speed variations
power loss under maximum acceleration
illumination of the TIL (this is the O/D light)

These concerns may also be accompanied by the following Diagnostic Trouble Codes (DTCs)

P0171 Bank 1 Fuel System too Lean
P0174 Bank 2 Fuel System too Lean (V6 models only)
P0172 Bank 1 Fuel System too Rich
P0175 Bank 2 Fuel System too Rich (V6 models only)
P1130 Lack Of HO2S11 Switch - Adaptive Fuel At Limit
P1131 Lack Of HO2S11 Switch - Sensor Indicates Lean
P1132 Lack Of HO2S11 Switch - Sensor Indicates Rich
P1150 Lack Of HO2S21 Switch - Adaptive Fuel At Limit (V6 only)
P1151 Lack Of HO2S21 Switch - Sensor Indicates Lean (V6 only)
P1152 Lack Of HO2S21 Switch - Sensor Indicates Rich (V6 only)

IMPORTANT UPDATE - 9/6/04

I have just replaced the MAF on my Windstar after discovering that Ford issued a TSB (Technical Service Bulletin) some time ago. Here's the symptoms and vehicles affected.

Article No.: 98-23-10
Date: 11/23/98
Mass Air Flow (MAF) - Sensor Contamination

Lack of Power.
Spark Knock/Detonation.
Buck/Jerk.
Hesitation/Surge on Acceleration.
Malfunction Indicator Lamp (MIL) Illuminated. DTCs P0171, P0172, P0174, P0175 may be stored in memory.

Ford:
1990-97 Thunderbird
1990-99 Mustang, Taurus SHO
1991-99 Crown Victoria, Escort, Taurus
1992-94 Tempo
1993-97 Probe
1995-99 Contour

Lincoln-Mercury:
1990-97 Cougar
1991-99 Continental, Grand Marquis, Sable, Town Car, Tracer
1992-94 Topaz
1993-98 Mark VIII
1995-99 Mystique

Light Truck:
1990 Bronco II
1990-97 Aerostar
1990-99 Ranger
1991-99 Explorer
1994-96 Bronco
1994-97 F Super Duty, F-250 HD
1994-99 Econoline, F-150, F-250 LD, F-350
1995-99 Windstar
1997-99 Expedition, Mountaineer
1998-99 Navigator
1999 F-250 HD, Super Duty F Series

Ford does not prescribe cleaning as a fix but only replacement.

After a second cleaning of the MAF my Windstar started knocking after just a few weeks. It has also been lightly surging/bucking at freeway speeds. I figured it was time to just replace the dang thing!

Thats cool.
Please be aware that I am not saying that cleaning your MAF is the cure-all of your issues/problems but that it simply will not hurt to clean it. Other than that, admittedly, I am clueless because I have never experienced what you have described. Replacing the O2 sensors might be a future option, as well.

i know it was mentioned before, but did you check that speedo sensor in the tail of the transmission? i unplugged mine after gear install before i got my tuner so i wouldn't rack up miles. once in a while when i would come to a stop it would very calmly idle down and die. strated right back up no problem. your case just reminded me of this. hope you figure it out.

ironically after installing my Bassani Catback I have been getting a System Too Rich Bank 1 code also. I had an exhaust leak before the passenger upstream 02 where my paper gasket blew out so I replaced it with a metal OEM gasket and cleared the code. Nevertheless, the code did return.

I also installed my JLT CAI 1-2k miles ago and I cleaned my MAF when I had it all apart so maybe it's time to clean it again and see how it works out for me. I am also going to check my speed sensor plug and I will get back with you and let you know if it works.

When are you getting the code? Mine only pops on when I am cruising at at least 60 mph.
