IIRC it was in Chicago.
Young girl was an extra on the set and almost lost her head when a cable snapped.

Yea that girl here in chicago from the first accident is paralyzed and brain damaged for life, she will never be the same again. She was a cute girl too. People in charge of directing all this aren't very bright if this keeps happening....
