Is this a Build Sheet?

I found this, what I believe to be a build sheet of some sort, since I've never seen one, I'm not sure. I found it up under the rear bumper. The torn off piece is unrecoverable unless I take apart the entire rear bumpper assy. It took me about 30 minutes to preserve this much. It felt like it was glued in place. Date at the top states May 21st. Any ideas? I will preserve this in plastic unless it's useless.

When i put my magnaflows on the other night, my brother spotted mine it was tucked into the coilspring in the rear. Anyhow great keepsake for collectors. Jerry

I had my car tagged as a show vehicle (No extra cost). The guy at the plant put all the build papers and check sheets under my spare tire (so the dealer wouldn't find them). I will get some pics if anyone wants to see them.
