Custom Performance, located in Concord, behind the Charlotte Motor Speedway. He did mine. Dale is the guy that owns the place.

There's piedmont, fastlane, competition auto (Fayetteville) all pro (also Fayetteville) those are the more or less "legit" shops in NC

Many shops charge different things. Gears are a little less than $200, the instal kit with bearings, etc can cost between $30-$100, whichever package you choose, and labor prices can range from $250-$400. I got lucky and found a reputable mustang shop to do mine for $250

I bought my gears from a friend, new for $100. I don't remember what Custom Performance charged, to be honest. It was some time ago.
