I picked this up used with the thought of adding to my brand new Aluminator build, but decided I did not want to bother with it. This is similar to the original "LDC" version that modifies two of the flow tubes (those are still around, but are about $175). I have no idea who made this, but it looks like it would work perfectly. Since the fitting for the driver's side head is basically the same size as the other, you'll have to bend the EGR tube to make this work. If you have no EGR, then even better.

This one is ready to go and the o-rings on each fitting look good. Only needs a section of 3/4" ID hose to connect the pipes. $100 shipped.
