Clutch adjustment problems

Recently installed a McLeod RXT a couple of months ago. I have a fiore firewall adjuster and upr triple hook quadrant and adjusted the clutch to where it engaged closer to the floor. Now the it engages way closer to the top of the pedal and I have no idea why. Also when I just barely put my foot on the clutch it sounds as if the TOB is rattling. Could it be the clutch cable is stretched out? Would like to get this fixed!

Did you use an adjustable pivot ball stud? Sounds like maybe the clutch fork came off the ball stud. Remove the lower inspection cover where the clutch cable goes through and take a look
