Citation Format question

Ok guys, I have heard that there are a few states that have citations that allow multiple violations to be listed with one citation (kinda like a ticket supplement). Do any of your respective states have that? If so, how exactly is it set up? I think it is a great idea so that you do not waste time rewriting the personal info over and over.

So is it just an attachment you add to it or the citation itself has room for multiple violations?

Both..........The citations we used had space to include four (4) violations, and an amendment form had ten (10) I believe, however it's been a few years. The two forms are pre-printed with your department's info, so all the amendment for needs is the serial number of the original citation.
