What shifter is this?

Bought a 99 cobra and he included some extras. He said this would fit the t5/t45 and was just wondering if possible exactly what model it is. It has no identifying marks on it either. If there is somewhere I should look to see what it is please let me know! Also is there anywhere to buy the 4 bolts to mount this if I can use it the bolts are m8 x 1.25 by 25mm long stainless steel I believe.

Top where the bolts go in through? Its just totally blank there should be 4 pictures showing.

The handle is a upr extreme handle, so I'm gonna guess that the base is a upr piece also.

Is there a way to see if it fits my t45 tranny or will I find out instantly when I go to install it? Know if this is a good setup as opposed to a steeda triax which is what I was going to buy.
