I had the problem return only about 3-4 weeks after I had it fixed under the TSB and it was determined that the fingers of the pressure plate were a little warped. Also the clutch disc itself was a little less than 1mm away from being completely worn also. I had them replace the pressure plate under warranty and the clutch disc as well.
