new clutch or throwout bearing?

I think the clutch is on its way out. if you ease out on the clutch then hit the gas it seems ok. if you ease out on the clutch,then pat the gas the car starts to jump.when you try 5th gear real fast it grinds. at 100m.p.h. going from 4th to
5th it grinds. i think the clutch & throwout bearing are bad. some opinions would be good . & the car is 01 mustang cobra, what is the best clutch set up & flywheel for this car.thanks for all the help.

to check the clutch for slipping put get the car rolling up to cruise speed in 3rd. then mash the gas and watch the rpms. if they rise up to fast your clutch is bad and you need to replace clutch, throwout bearing, and pilot bearing. if the trans is grinding it could also be the syncros in the trans and that would mean you need to have it rebuilt.
