Bad clutch disk

Hello all,
The clutch went out on my son's Focus SVT, we took out the tranny and the disk has NO splines. Looks like it was machined like that from the factory. See pic.
Does anybody know what could of caused this?

This has happened to me 3 times now, with 2 different tranys. Steve at Tasca(parts/com) can explain the original issues, but the first clutch Ford used was too small and upgraded. The LUK Rep Kit is the only one I've found and it is the correct size.
Steve pointed out the worn input shaft on my original tranny. Said that this was an issue with the clutch doing this.
My issues may be different than yours, but... If you've fixed it, or not, I'd like to chat with you about this. I love my SVT and want to drive it without a clutch every 7000 miles....
(My engine was rebuilt, and that's where I think my problem lies.)
