Had a similar issue and it turned out to be a vacuum leak. Unplug the MAF and start the car. if it starts easily and smooths out, that's probably your issue.

I had the bad hesitation between shifts and my tuner was able to almost completely eliminate it by tweeking something in the tune. What it was, I cant remember. Maybe someone can chime in.

Usually 10% difference is acceptable between cylinders. 5, 6, & 7 are a little on the low side. I'm assuming your car is probably a 2003? Some 03's had issue's with the drivers side head and the lack of coolant flow to it, it would get hot and cook the valve seals over time. Cylinders 7 and 8...
