1) A poorly engineered part.
2) Fan drawing too much current (faulty motor)
3) Idling too long with AC on in the AZ desert (for example)
4) Poorly fitting connection(s)
5) Bad Ground
Take your pick.. Seems like many things can cause this, but it all circles back to #1
